as far as i know simple queue is NOT multi-threaded this problem arise frequently when a single PPP interface plus another config when most traffic pass across that PPP interface Any alternative solution? FW upgrade fixed the issue. Load now spread evenly across the 4 cores.

70% load is high, I don't think that neither your PPPoE nor your NAT are causing it... There are currently 100+ PPPoE clients that are active in the RB and so is the same for the queues. Yeah currently, there are estimated there 10,000+ googlevideo domain. As of now my solution is using ip blocks where mostly my connection is made.

It's kind of expected for non-fasttracked traffic with queues, and as much mangle rules.
