ZeroByte, by analog telephone, im talking about telephone that is connected to analog card in asterisk pbx. So I asume that rtp NEED to go via port 3 to port 5, to reach PBX, and PBX is converting it to analog signal to analog phone can hear the caller. I know that rtp can co directly like p2p, but ...

I had the same issue and it was routing problem.
Returning connections were routed on wrong route and never reach originator.

If you think that the NAT is correct and the Forward chain has an entry permitting the traffic then double check that the target device is actually returning traffic to the RouterOS system - e.g. what is the target's default gateway set to?
