I would say you have a hardware failure on that server, perhaps cooling or PSU problems. What is the CPU load?

Have a Mikrotik server with 2.5Ghz P4 with 512M RAM. Peak times excedes 400 active PPPoE sessions. Uptime 24 days and the last reboot was due to a Mikrotik OS update. No problems here. Says 378M total RAM and 328M free so PPPoE does not seem to use much RAM. I would test your RAM. http://www.memtest86.com/

Are there any forms of pppoe ghosts showing up?
Are you using RADIUS or local user lists?
