Different MAC addresses, different hosts. One PPPoE session per host. Not one PPPoE session per username.

Or maybe you want to explain your perceived issue more completely?

In the profile I selected "only one", it should not be limited to only one User?

How do you assigned the IP address ppppe client? Is it from radius ?
If it's from radius, you need to work it from your radius.
I have experienced this issue, what I do is doubled checked from radius backend to SQL pool ip, if it's IP address has been used or not. When the backend radius to SQL slowed, this issue may be occured.

But the pool is from mikrotik itself. You need to check it to support.

I also want the mikrotik should disconnect the user if the IP address was duplicate.
