High CPU usage from "Queueing" despite having no queues

RB600A running v5.14. In Tools -> Profile, Queueing consistently shows about 30-40% CPU usage. However, we have no simple queues or queue trees. Interface queues are set to "only-hardware-queue."

RouterOS version you are running is over 4 years old, you really should consider updating it and see if that helps your problem.
