Adding terms and conditions checkbox to hotspot login page

Hey guys I have a hotspot config and I would like to use this Hotspot login page template
http://mikrotikthemes.airpoint.club/201 ... login.html
But I want to add a checkbox like "I have read and agree to the Terms and Conditions and Privacy Policy" and when you hover Terms and conditions text you can open the Terms and conditions
And if you dont check the checkbox you cant login the Hotspot
I dont know much about html and css ,
If is there anyone to help me about this I would be grateful
