VLAN on 750GL

I want to get some input on setting up VLAN's in RouterOS and perhaps some comments to see if I understand it correctly. Please refer to the attached image for my desired setup.
I want to have 2 VLANs:
VLAN 1 - 192.168.88.0/24
VLAN 2 - 192.168.89.0/24
VLAN 1 for PC's/NAS
VLAN 2 for VOIP
Only 1 device needs to be tagged in this setup (Phone 2)
My questions so far are this:
Do I need to create 2 bridges, 1 bridge, or do I not need a bridge at all?
Bridge 1 for VLAN 1 - Ports 2,3,5
Bridge 2 for VLAN 2 - Ports 4,5 (is this bridge required?)
Where do I apply the DHCP server for the 2 VLANs? Does this go on the bridge?
Am I missing anything else?
