Second 912 POE death

We just had our second 912 board die with failure of the POE adapter. It works fine powering it up using the power connector, just not over the onboard POE. Have others seen this? Any idea of what part is failing? The UART is fine, talks up a storm with power applied through the external connector, just something killing the POE side of the circuit... I have a feeling this might keep on happening and rather than throw away a perfectly good board ( some with AP licenses... ))...
Thanks if anyone knows more about this!
Robert

Are you using adequate surge protection?

Yes... And the ethernet port ( most likely to die from surges ) is just fine... The ability to take power over the ethernet port dies. This has only happened with 912 boards and nothing else on the same POE switches has died...
